The democratization of health refers to a transformative shift in the healthcare landscape, where access to health information, resources, and decision-making power is increasingly shared between healthcare providers and patients. This concept is pivotal in modern healthcare, as it empowers individuals to take an active role in their health management rather than being passive recipients of care.

The optometrist, a primary care provider and gatekeeper to the healthcare delivery system plays a key role in this transformative shift in the healthcare landscape. The eye is a window that can reveal invaluable insights into systemic conditions and vital organ and body health. The emerging field of Oculomics and the advent of noninvasive advanced imagining technology like the Color Fundus Photography, Optical Coherence Tomography (OCT) and Optical Coherence Tomography Angiography (OCTA) play an important role in the democratization of health by bringing into focus the Illness-Wellness Continuum and Salutogenic (Ease/Dis-ease Continuum and Sense of Coherence) holistic approach to health and wellness.

OCTA allows the optometrist to detect subtle early subclinical microvascular changes in the retina before clinical manifestations or standard systemic/organ-damage blood test biomarkers become abnormal. When the optometrist detects these subclinical microvasculature abnormalities, lifestyle changes that affect health, wellness, quality of life, and the trajectory of systemic conditions such as hypertension and diabetes can be discussed with the patient. With the aid of a health coach, these lifestyle changes can be implemented. Implementation of lifestyle changes can move a patient toward optimal health and wellness on the Illness-Wellness Continuum. The optometrist is in perfect position to assess whether signs improve, worsen or remain stable over time.

Key Aspects of Democratization of Health

Empowerment of Patients:

  • Patients are no longer just recipients of medical advice; they are becoming active participants in their health journey. This shift encourages individuals to take responsibility for their health by engaging in preventive measures and lifestyle changes.

  • This empowerment leads to a more engaged client who is informed about their health parameters and can make better decisions about their well-being.

Access to Information

  • The rise of digital health technologies has made health information more accessible. Patients can now monitor their health metrics through various devices and applications, which fosters a better understanding of their health status.

  • This access to information creates a  "mirror" effect, allowing individuals to reflect on their health and make informed choices.

Collaboration Between Providers and Patients:

  • The traditional model of healthcare, where the doctor holds all the knowledge and the patient is passive, is evolving. Now, healthcare professionals, such as health coaches, collaborate with clients to enhance health outcomes.

  • This collaborative relationship between the coach and the client is characterized by shared information and mutual engagement.

Focus on Preventive Health:

  • The democratization of health encourages a shift from reactive to proactive healthcare. Patients are motivated to adopt healthier lifestyles, which can prevent chronic diseases and improve overall health and wellness.

  • Understanding health parameters can lead to better lifestyle choices, ultimately affecting aging and disease prevention.

Sustainability in Healthcare:

  • By empowering patients and improving health literacy, the democratization of health can lead to reduced healthcare costs and improved outcomes. This is particularly relevant in the context of rising healthcare demands, costs and budget cuts.

Conclusion

In summary, the democratization of health is a significant paradigm shift in how healthcare is delivered and experienced. It emphasizes the importance of patient empowerment, access to information, collaborative relationships between healthcare providers and patients, and a focus on preventive health. This approach not only enhances individual health outcomes but also contributes to a more sustainable healthcare system. As we move forward, embracing this democratization will be crucial in addressing the challenges faced by modern healthcare systems and vulnerable at-risk patient populations.

Optometrists empower people through vision, helping them see a brighter future. Vision is not just physical. Vision is a physical, mental and spiritual phenomena of enlightenment. The word spiritual can also be interpreted as a salutogenic sense of coherence. Next to life itself, God's most precious gift is sight - the window to this mysterious thing called life. Not only does the eye serve as a window to this mysterious thing called life, but it also serves as a window to the systemic physical health of the body. As gatekeepers of the healthcare system, optometrists stand at the forefront, ready to guide communities through this transformative shift in healthcare.
